A stable pharmaceutical preparation for parenteral and ophthalmic administration comprises a water-soluble salt of a 21-phosphate ester of a 21-hydroxy steroid and a water-soluble saccharin in aqueous solution of pH 6 to 8. The water-soluble saccharin may be the alkali metal e.g. sodium, ammonium or water-soluble amine salts, all of which must be more water-soluble than saccharin itself. Optional ingredients are (a) antioxidants e.g. alkali metal sulphites, bizulphites and hypophosphites, alkali metal citrates, thioglycol, pyridine sulphinic acid, thiourea and sodium formaldehyde sulfoxcylate, (b) preservatives, (c) buffers and (d) antibiotics e.g. heomycin. Specified 21-hydroxy steroids are hydrocortisone, cortisone, prednisolone, prednisone, 9a -fluorohydrocortisone, 9a -fluoroprednisolone, 6a , 9a -difluorohydrocortisone, 6a -, 9a -difluoroprednisolone, triameinoline, triameinolone 16, 17- acetonide, 6a -methylprednisolone and dexamethasone. The water-soluble salt may be an alkali metal, ammonium or water-soluble amine salt.
