PROBLEM TO BE SOLVED: To obtain the subject medicine capable of sufficiently drawing out anti-rheumatic actions, by dissolving a cyclosporin and including the solution. SOLUTION: This medicine comprises a cyclosporin as an active ingredient. The medicine together with an arbitrary additive is dissolved in water (physiological salt solution, water for injection or distilled water) and sterilized to give an agent for direct intra-articular administration. A natural type cyclosporin A or C, their phosphoric ester derivative or its salt may be cited as the cyclosporin. The content of the cyclosporin is 0.01-35μg in the case of the natural type cyclosporin or its phosphoric ester derivative and 0.1μg to 500 mg/mL in the case of a phosphoric ester derivative salt of the cyclosporin. Consequently effects can be exhibited without adding an additive such as a solubilizer or by directly administering a cyclosporin in an extremely small amount in a range of solubility in water to an RA joint. Adverse effects by an additive and the whole body distribution of a cyclosporin can be avoided.
