PROBLEM TO BE SOLVED: To provide an information identification sheet having excellent forgery prevention and a method for identifying information. SOLUTION: This identification sheet is formed of two or more sheet layers. The sheet comprises a rectangular state having a width of 0.05 to 2mm, a thickness of 0.001 to 0.1mm and a length of 2 to 20mm in a paper layer adjacent to the outermost layer, and fluorescent splits each having a flat shape of 5 to 50 of a ratio of the width to the thickness are internally added to become random pattern. An ultraviolet ray having a wavelength of 200 to 380nm is emitted to the sheet surface to visualize the pattern of the splits in the support and read or recorded. The information of the pattern can be repeatedly identified.
