A pressure equalisation unit prosthetic replacement for a eustachian tube comprises an outer section (66) and an inner section (56), connected via a tube (54). The outer section can be anchored in a bone (76) and has an accessible region. It can hold a filter and has an internal connection (52) for a primary tube. The inner section has a housing which is divided into inner (60) and outer (58) chambers, sepd. by either a fine filter (63) or a pressure equalisation membrane. The inner and outer chambers have a connection (52) to the primary and secondary tubes.The tubes are pref. made of silicone.
