PROBLEM TO BE SOLVED: To provide a self-color forming sheet for optical read in which a color former mcirocapsule-containing ink layer for optical read is formed together with prescribed printing onto a sheet face in a collectively printing process and the formed shelf-color forming sheet for optical read inhibits blur of a coloring image for optical read more than in the conventional practice and precision of read is improved in the self-color forming sheet for optical read and to provided a method for forming said sheet. SOLUTION: The self-color forming sheet for optical read is provided with an ultraviolet curing type ink layer 4 in the prescribed part of a self-color forming face 3 of a self-color forming sheet 2 having the self-color forming face 3 coated with color former microcapsules and a developer. The ultraviolet curing type ink layer 4 is constituted by dispersing microcapsules c, which have a color former for optical read having absorption wavelength of a near infrared radiation region as core substance, in an ultraviolet curing type ink vehicle in such a state that a solvent D with a dispersantlike function having affinity for the ultraviolet curing type ink vehicle V and capsule C wall material is interposed between the vehicle V and the capsule C wall material.
