<p>PROBLEM TO BE SOLVED: To realize formation of a large quantity in a short time simply by a low cost by simultaneously printing photographed images, for example, human face or upper part of his body and other information on an image print medium as an image printing adhesive sheet. SOLUTION: This forming apparatus comprises a CCD camera 20 for converting an optical image into an image signal, a monitor 22 for displaying the signal output from the camera 20, and a video printer 24 for printing the signal output from the camera 20. In this case, the signal is printed by the printer 24 on a cut area of the image print medium formed of a base part having a print on one surface and pressure sensitive adhesive coating the other surface and a release sheet laminated on the base part with the adhesive and having the cut area formed by previously cutting the print surface and the base part in desired shape.</p> |
