This invention concerns a form which has an integrated card which can be separated from it. Parts (K1, K2) which overlay and adhere to each other in the card are arranged, in part, in the opposite order in the form. By turning in place and/or by laying one on the other in solely one direction, they can however still be glued to one another. At least one of the two card surfaces can be written on in the form. The writing on the completed card is covered by another layer and thereby optionally sealed. To produce the form, it suffices to apply a preassembled multilayer material to one side of a sheet form and to carry out at least one stamping (S1, S2) in the area of this material. However, the preassembled material can also already include the material of the sheet form. The cards of the invention can be punched completely, i.e. without residual parts, from the form and are, insofar, simple to separate. |
