The axis of the passage (30) and its distal opening are offset from its tip (31). An insufflation needle (3) is slidable in the passage, and has a lateral outlet (5) for the insufflation medium. The needle can slide in opposition to an elastic force, its distal end protruding beyond the tip in the unloaded state and being retracted by an external force. Its distal end may be blunt or conical, and flush with the trocar surface in the retracted position. The trocar tip can be chamfered to form cutting edges, the passage outlet being in one of the chamfers.
