The bone anchor (2) has a shank section (202) and a head formed as a fork defining two branches (201'") receiving between them the rod to be fixed. A locking screw (5) is threaded between the branches and a threaded sleeve (4) is threaded onto the rod and interposed between the base of the seating and the screw. An angular locking cap (102) fits into the gap between the screw, a collar (6) and by the edges of the two branches of the U-shape. the angular locking cap can come into contact with the top of the rod and is held by radial engagement between the collar and the screw.
