An apparatus (10) is used to perform ocular surgery. A fragmenting tip (14) is simultaneously rotated and reciprocated ultrasonically such that tissue is fragmented by the combined motion of a fragmenting surface perpendicular to the tissue, and at an angle to the tissue many times during a single revolution or part of a revolution. This will reduce damage to surrounding tissues while fragmenting other tissues, such as the capsular wall during removal of the lens during cataract removal surgery.
