PROBLEM TO BE SOLVED: To inhibit information entry spaces from being recorded on the receiving form, and carry out optical reading without detriment to the information reading by rendering information entry spaces recognized visibly clearly during the period of entry of optically reading information, and avoiding the existence thereof during facsimile transmission. SOLUTION: The optically reading facsimile transmission form 1 is formed that an ancillary sheet 3 having information entry spaces 5-12 to be written in copy to optically reading information copying entry parts is separably bound on the upper part of a facsimile transmission sheet 2 having optically reading information copying entry parts. Also, there is provided a noncarbon type copying structure for writing down articles written in the information entry spaces 5-12 of an auxiliary sheet 3 in the optically reading information copying entry parts of the facsimile transmission sheet 2, and in a part corresponding to the optically reading information copying entry parts of the facsimile transmission sheet 2, timing marks are put for optically reading the optically reading information to be written in copy. |
