PROBLEM TO BE SOLVED: To enhance productivity by providing a groove-shaped recessed part to a fiber lens array and providing a ridge-shaped protruding part to the lens attaching part of a case and fitting the groove-shaped recessed part and the ridge-shaped protruding part to each other to attach the lens array to the case. SOLUTION: A center position P is set to a fiber lens array 5 and groove- shaped recessed parts 5b are formed to a rotary shaft at a predetermined interval A so that the center position P coincides with the center of the interval A. Ridge-shaped protruding parts 2d are provided to the surface opposed to the side surface provided with the groove-shaped recessed parts of the fiber lens array 5. After a lid part 2b and the fiber lens array 5 are formed, the ridge-shaped protruding parts 2d are fitted in the groove-shaped recessed parts 5b and the fiber lens array 5 is attached to the lid part 2b. By this constitution, the fiber lens array is accurately attached at a planned attaching position and the movement of an image forming position can be practically neglected.
