The image prodn. unit (10) includes an ultrasonic transmitter and an ultrasonic receiver. The ablation unit e.g. cutting unit has a rotating drilling or milling head (4). The drilling or the milliner head has a variable dia. The ablation unit has a unit for emitter laser light. The ablation unit is movable, by the use of a first guiding system (12) introduced in an opening (13) of the vessel narrowing (2), in the direction to the vessel narrowing. The first guiding system has a guide wire (12), on which the ablation unit is movably arranged.
