An optical data recording/reading medium comprising at least one transparent layer (2) for recording or reading out data on various planes (S1-S3) in the body of the medium. The medium comprises one or more data items (5) recorded on a main plane of at least the transparent layer for providing track following and/or focusing and/or timing and/or addressing functions. The medium further comprises one or more guiding data items (4) recorded in each plane. Said medium may be used as an optical disk with recordings on multiple planes.
