The hand piece for medical an instrument, such as forceps, has two counter operating grips (1,2) which act against spring pressure to operate the instrument. A ratchet arrangement (7) acts to secure the grips in at least two positions against springing back. At the end position the grips are automatically unlocked. Between the locked positions (20) of the ratchet, there are positions (21) where the ratchet is automatically unlocked. The ratchet arrangement has two arms (8,9) which each join to the counter operating grips and are joined about a common axis (11) which is parallel to the main axis of the grips.
