A process for the manufacture of Diltiazem of the structural formula (see fig. I) or deacetyl-Diltiazem of the structural formula (see fig. II) which comprises the use of methyl isobutyl ketone (MIBK) as solvent and sodium hydroxide (NaOH) as base in water.
