<p>PURPOSE: To eliminate the matching work of the destination printed on an ordering postcard form having a postcard piece used in the case of ordering a commodity by mail-order selling to the fill-in content of an invoice. CONSTITUTION: The ordering postcard form 1 comprises an ordering postcard part 4 having two postcard pieces 9, 10 conforming to the standard specified as the postcard and a general information display piece 11, and a delivery information description part 5 having a delivery information describing piece 13 and individual information display piece 14. A membership number 24, name 25 and address 26 of ordering person information are displayed and an ordering commodity fill-in column 23 is provided on the front surface sides of the pieces 9 and 10, the commodity ordering destination is displayed as the address on the rear surface side, further a delivery and receipt cut form 16 and an absence advance note 18 are provided on the piece 13, and a sending address 36 of the ordering person sending destination is displayed on the consignee fill-in column 17 of the form 16. Since both the pieces of ordering person information 24, 25, 26 and the address 36 are displayed on the form 1, the matching work is eliminated.</p> |
