FIELD: operative stomatology; combined osteosynthesis of linear and multi-comminuted fractures of mandible, except for gunshot fractures. SUBSTANCE: method involves inserting of fine metal intramedullary pin, putting bone stitch with shaping hole in the fractured fragment for ligature, compression of fragments, followed by removing ligature and intramedullary pin. Intramedullary pin is inserted into bone from the lower edge of jaw, with free end of pin being projected beyond near edge of jaw. Then, one hole is drilled in distal fragment of bone, ligature is passed through this hole, compression is created by tensioning and attaching ligature to free end of pin. When consolidation is completed, ligature and intramedullary pin are removed after cutting along projection of free end of pin by simultaneous and successive pulling out ligature and pin. In addition, intramedullary pin is inserted from the lower edge of medial fragment into distal one along "frontal-backwards-upwards" direction. The pin is entered so that 3.0 - 3.5 cm of its length is inside bone on both sides from crack of bone. Length of free end of pin is 0.8 - 1.0 cm; ligature is tied at apex of angle between a pin and lower edge of mandible. EFFECT: improved strength of osteosynthesis by using reliable immobilization of fractured fragments and their compression; simplified procedure of putting bone stitch and removing ligature and intramedullary pin after consolidation. |
