<p>PURPOSE: To provide an identification card forming apparatus in which the quality of an original in safekeeping can be improved by largely alleviating the labor hour for forming an application form by the applicant of a license, and printing the correct face picture and describing information occupying most of the form together with the license on the form. CONSTITUTION: A face picture imaging unit 10 prints the face picture of a person to renew from a video camera imaged to form a license on the print area of an application form. An application form forming unit 12 prints the described information of the surface formed by communicating with a host computer 1 to form the license on the form, further images the picture of the form formed in this manner on both side imaged parts as it is, and stores the face picture and a license number in a picture filing unit 14.</p> |
