FIELD: medicine; surgery. SUBSTANCE: method involves application of endotracheal narcosis for performing right-sided transrectal laparotomy. The transverse colon is displaced upwards, and an opening is made in the mesentery where there are no blood vessels. The opening edges are sutured to the front wall of the duodenum, and an empty intestine loop located near the Treitz ligament is led to the duodenum. Two sero-muscular sutures are applied to fix the empty intestine loop on the upper front wall of the duodenum. An intestinal "spur" is formed and the duodenum wall is vertically dissected below this spur. The dissection length is 12-15 mm. The leadaway part of the empty intestine loop is opened, and one row of single stitches is applied on the anastomosis front and back lips so that their nodes are in the intestinal lumen. The front abdominal wall is sutured layer-by-layer. EFFECT: blind loop syndrome prevented. |
