PROBLEM TO BE SOLVED: To obtain the subject phthalidyliden esters, having characteristics capable of blocking or antagonizing a tumor necrosis factor, and useful for treatment of endotxic shock. SOLUTION: This pharmaceutical composition for treating endotoxic shock contains (3-phthalidyliden)alkyl esters of carnitine of the formula (Y is a 1-5C alkylene non-substituted or substituted with one or more of 1-4C alkyls; R is H or a 2-6C alkanoyl; R1 to R4 are each H, a halogen, a 1-8C alkoxy, a 1-4C alkyl, a lower alkyl substituted with a halogen, amino, a 1-4C alkyl-substituted amino, NO2 , CN or a l-4C alkanoylamino, or R1 and R2 , R2 and R3 , or R3 and R4 form a 1-4C alkylenedioxy; X<-> is a pharmaceutically acceptable acid anion) and alkanoyl carnitines, e.g. (Z)-[3-(5-chlorophthalidylidene)ethyl] ester of isovaleryl-L-carnitine bromide as an active ingredient. The daily dose of the compound is most suitably within the range of 10-50 mg/kg body weight.
