PURPOSE: To provide a forming method simple and accurate in forming procedure releasing the complicatedness at the time of the formation of a medical expense supply application having two or more kinds of formats. CONSTITUTION: An individual code number is printed and displayed on the rear surface of each of receipt papers 1 as a bar code 2 and receipt papers 1 are superposed and set on the hopper part 4 of a printer 3 at random. The bar code 2 of the lowermost receipt paper is read by a bar code reader 5 and, by the input of the reading signal of the bar code reader, the medical expense supply application content corresponding to the individual code number is issued from a computer. The medical expense supply application content is printed on the receipt paper 1 sent to a printing part 8. The printed receipt paper is successively stacked on a stacker part 10.
