<p>PURPOSE:To simplify the work for reading clinical records by composing the clinical records of one card respectively provided with columns for respectively describing a patient ID, the date and time of a description and the result. CONSTITUTION:Clinical records K are composed of one card provided with an ID description column 10 for describing the patient ID specific to each patient, date and time description column 11 for describing the date and time when the clinical treatment result composed of characters showing the disease name of the patient or the symptom name, for example, and a human body figure concretely expressing the symptom, result description column for a doctor to describe the clinical treatment result, and marking column 13 continuously provided from the head to end of the clinical records K so as to describe an end mark M. Therefore, each time the treatment or examination is once finished, for example, it is enough only to read the result so that it can not be necessary to use one card twice. Thus, the work for reading the clinical records K can be simplified.</p> |
