PURPOSE:To make it possible to easily connect an optical waveguide and an optical fiber with a simple structure with low loss and high accuracy by integrally forming a first guiding groove for positioning of the optical waveguide and the optical fiber on an optical waveguide substrate. CONSTITUTION:The optical waveguide substrate 11 has a substrate part 11A and an optical waveguide forming layer 11B formed with the optical waveguide (core part) 12. The first guiding groove 14 for positioning the optical fiber 13 onto the extension line of the optical axis of the optical waveguide 12 adjacently to the end (end face) 12a of the optical waveguide 12 is formed on the substrate part 11A. The fiber substrate 15 is disposed in correspondence to the first guiding groove 14 and holds the optical fiber 13 cooperatively with the first guiding groove 14. The surface part 11a of the optical waveguide substrate 11 and the surface part 15a of the fiber substrate 15 facing each other are fixed in the state of holding the optical fiber 13 fixed to the fiber substrate 15 in tight contact with the first guiding groove 14 on the optical waveguide substrate 11. As a result, the optical waveguide 12 and the optical fiber 13 are connected.
