The handpiece (1) focusses laser light from a light guide (3), centred in a joint between a supply tube (2) and the handpiece. The light guide is longer than part of the handpiece, displaced eccentrically towards the inside of the handpiece elbow (7). The joint (9) between the supply tube and light guide, rotates in a housing (8), in the handpiece. The light guide end at the side of the handpiece may be orientated obliquely, relative to the longest axis of the handpiece. Where the handpiece has a small elbow with the head fixed to the handpiece, the light guide may extend adjacent to the head mirror (11). The handpiece may include an electronic unit (6) and the light output (5) is from the head.
