A form tie (1) for interlocking parallel and spaced foam panels (27). The tie (1) is formed of an elongate planar strap (2) having a lattice arrangement of openings (8) for flow of concrete. Each of the opposite ends (3, 4) of the strap (2) have an end plate (18) and an inside plate (16) spaced to bracket a corner of a foam form panel (27) with each of the plates (16, 18) extending in a perpendicular or "Y" axis direction to the longitudinal or "X" axis of the strap (2). Each of the end portions (3, 4) forming the bracket (25) also has vane shelves (20) extending between the end and inside plates (16, 18) in a "Z" axis direction to the strap (2) for support of the foam form panel corner. "U" shaped holders (12) in the edges of the strap (2) and overcenter tabs (14) in the openings (12) provide retainers to hold reinforcement bars (29), commonly termed rebar. The strap (2) is preferably formed of a heavy gauge polypropylene.
