PURPOSE:To embody the efficient utilization of medical treatment data and the simplification of business. CONSTITUTION:A diagnostic recording material 1 is constituted of sheets 3, 4 formed by folding one paper along a fold line 2 and the chart 5 for recording consultation progress held between the sheets 3, 4 and a bag like cover 6 is provided to the sheet 4. The chart 5 of which the insertion part 5a is formed into a tapered shape is received in the cover 6 and taken out of the cover 6 at every consultation progress and consultation progress data is printed by a printer 15. The chart 5 completed in printing is bonded to the sheet 3. Medical treatment data can be efficiently recorded on the chart 5 using the printer 15 used accompanied by the introduction of a computer and the control of the chart 5 is easy to perform. |
