The surgical instrument has a peripherally perforated tubular shaft (1). A tube (2) is fitted on the outside of the shaft covering the peripheral shaft holes. The outer tube is inserted for the actual surgery. For cleansing after surgery a nut (3) or a bayonet closure is loosened and removed together with the outer tube. The instrument has a distal end handle (5). The design permits proper cleaning and sterilising.
