The present invention relates to a piston internal combustion engine driven by a cam, including a cylinder, a piston, an intake and exhaust mechanism, an ignition system, a transmission pin mounted in said piston and a cylindrical cam. It transforms horizontal motion of the piston into rotation of shaft by cooperating said transmission pin with said cylindrical cam, so that the conventional crankshaft and connecting rod transmission mechanism can be substituted. It is an engine with compact structure, small size, light weight and reliable and smooth operating.
