The present invention provides a tongue positioning apparatus (10) with a receptacle structure (10a, 10b, 12, 13, 14, 15, 16) configured for reception and retention of the outer extent of the user's tongue, airway structures (8) in the receptacle structure, a shield structure (36) shaped to be received and retained on the tongue receptacle structure and positioned just forward of the user's lip or lips, and an attachment structure (92) for adjustably attaching the shield structure to the receptacle structure to permit selective adjustment of the position of the receptacle structure relative to the tongue forward extend whereby the tongue may be comfortably positioned and retained in the receptacle structure, and whereby the tongue is brought forward. Incremental forward movement of the shield will move the tongue further forward, lessening any airway obstruction. A preferred receptacle structure includes one or more tubes (80) to facilitate movement of air between the user's lungs and the outside. One preferred form includes large diameter tubes (80), designed to admit and pass endotracheal tubes and surgical devices. Another preferred receptacle structure includes a sealable, vacuum connection (95).
