PURPOSE:To provide a device which are accessed from many terminal equipments including telephone sets, in which the reservation for medical treatment or the like is automated in hospital or a clinic or the like and man-hours of a management job of the hospital are smoothly saved and the service to diagnosis reservation personnel is improved. CONSTITUTION:The reservation of medical treatment is attained by an entry unit 30 able to enter numeral data or the like from a telephone set or the like and a reservation unit 40 setting the reservation to a reservation file 51 or the like of a file unit based on the entered data without visit to a hospital and also attained without intervention of hospital staff. Moreover, a diagnostic state is displayed on a display unit 60 based on the reservation file 51 to provide convenience for reservation personnel awaiting the medical treatment. Furthermore, the man-hours of a hospital job are saved by providing a function 70 selecting a carte and sending the selected card based on the content of the reservation file 51.
