<p>PURPOSE: To obtain a simple and prompt exchange of seals by installing at least one support member for seals with a small diameter in addition to seals with a large diameter installed at the end on the distal side of a patient relating to a cannula sleeve equipped with a valve, etc., for operations under an endoscope. CONSTITUTION: At one end, a cannula sleeve 1 has a valve 7 consisting of a flap valve into which an instrument can be inserted and a connection part 8 for rinsing liquid or an air passage, is protruded in the radial direction from the valve 7. The valve 7 is operated to open and close by a switching lever 9. Relating to such a cannula sleeve 1, at the end of the distal side of a patient, a cap type rubber seal 2 to pass an instrument with a relatively large diameter, is installed. At the outer end of the holder 6 placed in the range of the rubber seal 2, a support member 3 having a hermetically sealed cap (a seal with a small diameter) 4 is placed to fluctuate freely by way of a joint 5 to pass a surgical instrument with a small diameter.
