The instrument has a first guide limb (1) and a second guide limb (2) which start with their proximal ends (3, 4) from a connection part (8) and whose intermediate area (10) is accessible from their distal ends (5, 6). A guide member (20) is mounted in the first guide limb (1) and/or in the second guide limb (2).The guide member fills a part of the intermediate area (19) in its middle or proximal zone. The guide member (20) is designed and mounted in such a way that a thread (F) introduced transversely in proximal direction into the intermediate area (10) is inevitably guided by means of the guide member (20) essentially along the first guide limb (1) until it has passed a guide point (P) lying on the longitudinal axis (L-L) of the intermediate area (10).
