A form arrangement 10 having a bag 11, in which a sample of blood, urine or other substance may be placed, a form 12, on which information relating to the sample may be written, and sealing means 13 for sealing the bag, has a detachable label 14 with an identification marking on it, assembled with the bag, form and sealing means. One or more labels, with common identification in the form of numerals or bar code, may be adhesively secured to a backing sheet 17. The backing sheet may be attached to the form by mutually abutting edges or adhesively to the face of the form into which the labels have been cut or onto which the labels have been tabbed. The bag may be attached directly to the form or to the backing sheet. The sealing means of the disclosed embodiments is a strip of adhesive with a detachable cover strip over it so that when the cover strip is removed the form or bag may be folded to close the opening on the bag. The arrangement reduces the risk of the form and sample becoming separated, or of mis-matching samples and information. <IMAGE> |
