PURPOSE: To provide a multi page balancing accounts slip and its forming method for forming a multi page mailer capable of providing easily balancing accounts and being bundled into a booklet after being unsealed. CONSTITUTION: The identification information is printed on a top face of a top sheet 10, a perimeter area of a predetermined width is formed along each peripheral edge of a bottom face, an adhesive pattern as a first adhesive pattern is applied to the perimeter area with a width approximately half of the width of the perimeter area, and the perforation is applied inside of the adhesive pattern. The identification information is printed on a top face of a bottom sheet 13, and an adhesive pattern as a second adhesive pattern which is not aligned when it is overlapped to the bottom face of the top sheet 10, is applied within a perimeter area. The perforation to be aligned to the perforation is applied inside of the adhesive pattern. The identification information is printed on a top face of an intermediate sheet 12, the adhesive patterns 33, 34 of the first adhesive pattern are applied to the perimeter areas, the second adhesive pattern is applied to the bottom face within the perimeter area, and the perforation to be aligned to the other perforation is applied inside of the adhesive pattern. |
