An instrument tube (4) whose distal end is insertable into the abdominal cavity through a trocar sleeve. In the gripper (2) main body (10) is contained an actuator (30), longitudinally slidable within a preset range against a spring (16) force. The main body has a connectable shaft (24) in the instrument tube. In the shaft is a longitudinally slidable actuating rod (22) coupled to the actuators. Near the actuating rod distal end is fitted a holder for ratchet head (8), matching a ligature binder with toothed end region. Near the shaft distal end is fitted a second such ratchet head (9), closer to the instrument tube distal end.
