A spreading tool (E) has an end portion shaped to receive and guide a sheath (F) into the carpal canal. The sheath receives an optical endoscopic probe and a cutting instrument (C) whose blade portion is of adjustable height relative to the thickness of the ligament to be treated. The spreading tool is of scissor-type construction, with a pair of pivoted arms (1,2) including jaws (1b1,2b1) for spreading the skin to allow passage of the sheath. A rack mechanism (4,5) between the handle portions (1a,2a) of the tool ensures adjustable locking at a predetermined spacing. ADVANTAGE - Minimises skin incision, reducing scarring and trauma during and after surgery.
