The sleeve tube (12) encloses the surgical instrument (16) which is axially movable from a rest position in the tube to a free position and also into an operation position in which it is fixed. The surgical instrument is a blade (16) and the sleeve tube together with the rod (20) carrying the blade are displaceable relative to one another and/or rotatable. The free position of the blade is its cutting position and the cutting edges (17) run approximately parallel to the longitudinal axis (A) of the sleeve tube or can run at least partly at an angle to the longitudinal axis of the sleeve. They have a U-shape, the U-arms in the cutting position running at right-angles to the longitudinal axis.
