<p>PURPOSE:To accurately fix a form and to easily and surely additionally write data on the form later by detecting that the form is positioned on a tablet by holes. CONSTITUTION:The hole of a tablet enclosure 3 and a hole 2a of a clinical chart 2 are aligned to position anf fix the chart 2 on a tablet 1 by a positioning tool. Or a projection 8 of the tablet enclosure 3 and the hole 2a of the chart 2 are aligned, and the chart 2 is positioned and fixed on the tablet 1 so that the projection 8 is inserted through the hole 2a by a pressing plate 10, and a detection switch 13 is turned on. The mode is automatically switched to the handwriting input mode by output of a detection signal 86 indicating that the chart 2 is positioned and a detection signal S7 indicating the existence of the chart 2, and data corresponding to entered contents is recorded in a memory 31 as it is, and it is easily and accurately discriminated whether the chart 2 is correctly set or not.</p> |
