For construction of underground concrete structures, e.g. manholes or water collector tanks, the form is assembled of prefabricted regin components including inner and outer panels (100,200), spacer-connectors (300) and top and bottom plates (400,500). The inner and outer walls are assembled of respective component panels each having side ears for bolting joints and are spaced with the spacer connectors to be supported on the bottom plate and covered with the top plate. Concrete is poured into the cavity between the walls, which either remin set with the concrete or is disassembled after curing of the concrete for reuse of the panels. Prefabrication and assembling of resin components reduces the cost of form construction to the advantage of leaving the form with the concrete construction instead of disassembling it for reuse.
