A recorded data reader is provided having a light source for emission of a beam of light to a recording medium and a photosensitive means. A polarized beam splitter incorporate an interface developed between a titanium diffused layer and a proton exchanged layer formed by treating one region of the titanium diffused layer for proton exchange, both arranged on the substrate of lithium niobate material. Also, a collimator lens, a surface wave electrode, and a grating lens are embedded in the same substrate thus forming a means for transmitting and separating beams of light.
