PURPOSE:To enhance the efficiency of medical diagnosis by bonding the seal corresponding to the time and region of diagnosis and treatment to a card and entering a required item using the standard figure on the seal. CONSTITUTION:A large number of seals showing the dogteeth, incisors and molars of the upper jaw, the dogteeth, incisors and molars of the lower jaw, a distal surface, a mesial surface, a lingual surface and a labial surface are bonded to one sheet at every seals and the sheets are gathered in the number corresponding to the kinds of the seals to constitute one seal book. A doctor places the seal book by a treatment desk and peels off the desired seals from the seal book at the time of the entry of clinical charts and bonds the seals to clinical charts 1, 2, 3, to use the same. Required items can be entered in the clinical chart simply and accurately and the labor at the time of the entry of the clinical chart is reduced and the clinical chart can be read rapidly and accurately and, therefore, the working efficiency of medical diagnosis can be enhanced. |
