The adapter comprises a rigid shell (166) having an enlarged end portion (168) for snap-engaging over the mouth (170) of a drug vial (162). The opposite end of the shell is adapted for engaging a receptacle (42) having a pierceable situs (80) and connected in an administration set. The shell slidably houses a plate (176) mounting a hollow inlet cannula (180) and a hollow outlet cannula (182). the cannulas can pierce the situs (80) in the receptacle and are also movable, by sliding the plate (176) to pierce a stopper (172) of the vial (162), so as to provide for fluid flow from the receptacle (42) through the inlet cannula to the vial (162) and from the vial through the outlet cannula to the receptacle.
