<p>A thin film (13) of molten resin extruded from a T die (12) is pressed and cooled with a chill roll (14) equipped with a hologram relief master plate (17) on its peripheral surface, whereupon a hologram pattern is simultaneously formed as an integral part of the resin sheet being molded. The resin may be a polypropylene having a Rockwell hardness of at least 70, amorphous polyethylene terephthalate, an ethylene/vinyl alcohol copolymer, or a polyethylene resin having a Vicat softening point of at least 90 DEG C or polyamide. A reflecting metal layer may be over the whole or part of the hologram pattern forming surface. The sheet may form part of a laminate containing a substrate layer sealant or heat fusible layer, protective layer, heat resistant layer, adhesive layer, primer layer, paper layer, release paper or tackifying layer, (see for example Figures 5, 13, 14, 15, 24, 25 and 29).
