An instrument (10) for use in knee surgery comprises a tool carrier rail (12), a locater arm (13) mounted on the rail and projecting inwardly therefrom so as to be able to be introduced from the front of the knee and to a predetermined depth so that the free end of the arm is located at a required position within the knee, and a tool guide (15) adjustably mounted for movement along the rail (12) to any required angular spacing from the locater arm (13), the guide (15) serving to mount a surgical tool which can be operated to carry out any required lateral entry knee surgery and with the length of the locater arm and the mounting of the locater arm and of the guide being such that the surgical tool can be driven along an axis which substantially passes through the free end of the locater arm. <IMAGE>
