The invention relates to a set of forms, being composed of a printed sheet (2) which has at least two transverse grooves or perforations (3 and 4) at a distance from one another, the groove (3) forming a top flap (5) and a bottom flap (6) which are preferably of equal widths, and the groove (4) separating a pocket flap (7) from the bottom flap (6), the pocket flap (7) being folded back and the flaps (6 and 7) being connected, e.g. glued, to one another at at least one lateral edge (8), a snap-out set (13) being glued to the inner surface (12) of the top flap (5) and the sheet (2) being a supporting web sheet or a supporting single sheet. <IMAGE>
