PURPOSE:To form a sheet with ordinary paper by reading a bar code signal by a main scanning in the row direction by use of an optical device, starting a new row by a width scanner in the column direction, and inputting the result to a computer system. CONSTITUTION:During the main scanning in the normal direction by a carriage 27, the light projected from a light source lamp 28 and reflected by a code pattern sheet 1 is received by a photoelectric converting element 30 through a focusing lens 29 and the bar code signal on the sheet 1 is read by the photoe lectric converting element 30. The carriage 27 is reciprocated over the whole width of the sheet 1 by rotating clockwise and counterclockwise a main scanning motor 34, and the main scanning is attained. When the carriage is reversed in direction by switching the motor, a motor 23 for sub-scanning is actuated to turn a platen 21 by the share of one row to start a new row. A decoder 43 converts the bar code signal into a predetermined code and the result is fed by a serial/parallel converter 44 through a connector 15 to the computer system.
