The invention relates to a unit for ultrasound-directed endoscopic diagnosis and therapy for the treatment of pathological changes in the brain cavity along an access route created by trephination. A guide tube which can be advanced by means of a puncture is divided into mutually independent channels, is mounted displaceably outside the zone of operation and is designed to deliver liquid. The intention of this is to reach the target area extremely precisely through a miniaturised access route. This is achieved by a telescopic-type ball bearing traveller in which the guide tube can be moved axially and to all sides being assigned at the access opening to the cranium. The guide tube is divided by a middle bar into two channels for supplying an irrigation solution and returning, and other channels are accommodated floatingly in the channels and are used for receiving conduits and probes.
