PURPOSE:To suitably form a report in various forms on an examination results by reading an examination receiving slip filling medical examination data with hand written characters by an optical character reader and storing it in a computer and executing an information processing. CONSTITUTION:When the medical examination is executed, the result is written in the relevant column of the examination receiving slip 6 on a medical history or the like from the contents of an inquiry slip according to a copying or the like and filled with numerals by a pencil as the medical examination data. This is sent to a computer processing center and the hand written and filled medical examination data is read by the optical character reader 4. Then, the medical examination data is updated and processed by the memory device 8 of the medical examination data. Then, a check list is punched out, checked in a medical examination means and when it is erroneous, the examination receiving slip 6 is rewritten. If there is no error, a table forming is asked finally, the report is formed in the computer processing center, stored and distributed. In such a way, general medical examination data is tabulated to easily manage health.
