A manifold form assembly (2) has a front page (4) and a back page (18), with each page having image-receiving areas disposed thereon. The front page image-receiving areas (14,16) are non-overlapping with the back page image-receiving areas (24,26). A pressure-sensitive image-transferring system is provided integral with the assembly (2) to transfer pressure-created images formed on the front (14,16) and back page image-receiving areas (24,26). Duplicate front (42) and back pages (56) receive images transferred by the image-transferring system. A stub (6) connects the original pages, the image-transferring system, and the duplicate pages into an integral form assembly (2). |
